WHO IS ELIGIBLE TO APPLY?
Patients and families receiving active long term treatment (2 weeks minimum) in TMC medical facilities are eligible to apply.

HOW DO I APPLY?
Apply Online here. A volunteer will call you during regular hours to confirm your information.

Apply by telephone by calling the CAM Telephone Volunteer Office at 713-524-2640.
Volunteers are normally in the office Monday through Thursday between 9:30 a.m. - 12:00 p.m. and 1:00 p.m. - 3:30 p.m.

You may apply at other times by leaving a voice message. Please speak slowly and spell your name. A volunteer will call you during regular hours.

WHAT INFORMATION IS REQUIRED ON THE APPLICATION?

  • Patient’s name
  • Patient’s Hospital ID number
  • Home, work and family phone numbers
  • Houston contact numbers – hospital, hotel, friend
  • Social Worker’s name and phone number
  • Date apartment needed –DON
  • Estimated length of stay – LOS

WHAT DO I DO AFTER I APPLY?
You will need to keep your application ACTIVE by calling the Volunteer Office periodically. Any change in your application should be reported promptly. Routine contact to confirm your continued need and to confirm that your application is ACTIVE should be done on Tuesday through Thursday every 2 weeks or more frequently. Applications where there is no contact for a month are reclassified as INACTIVE. If you leave your information on the answering machine, please provide your NAME and PATIENT ID.

We expend considerable effort to maintain your application. If your plans change or you find other housing, please assist us by notifying us to remove your APPLICATION.
